Cambs GP Network are now offering a Home Visiting Service for housebound Diabetic patients who have been referred to us by their GP Surgery. The service is a Health Care Assistant led service targeting those patients who have not had their 8 care processes completed.
As part of the Diabetic Review the HCA will perform a foot check, blood tests, height and weight checks, blood pressure checks and obtain a urine sample.
The team will record their findings and escalate urgent medical concerns back to the patients GP Practice on the day and any other escalations sent back through the discharge summary.
This service is proving to be a valuable service for patients within the CGPN footprint.
